MIDDLETOWN, NY - 05/04/2018 — Cantex Continuing Care Network was among a select group of finalists who received a 2018 DecisionHealth Platinum Award. The awards recognize individuals and organizations working to ensure a “safe, quality and sustainable health care system” and to enhance the patient experience across the continuum of care.

Components of the calls include confirming attendance at follow-up PCP appointments, ensuring prescriptions have been filled, addressing key areas of health literacy and assisting with the management of chronic conditions. By working in close coordination with the internal Cantex team, these patients receive the highest quality of care. Since implementing the program, Cantex has seen a positive impact on preventing readmissions after residents have returned home, as well as a significant increase in revenue.
